Bloodless HeartMate 3 explantation and orthotopic heart transplant in a heart failure patient refusing blood products

      A 39-year-old man with end-stage heart failure from idiopathic non-ischemic cardiomyopathy underwent successful bloodless HeartMate3(HM3) implantation. Due to excessive out-of-pocket costs and obesity, he initially was deemed not a transplant candidate. He tolerated LVAD therapy until he was admitted 24 months later with recurrent ventricular tachycardia and ICD shocks. During the interim he lost 100 pounds and switched insurance plans. He was re-evaluated while inpatient and listed for heart transplantation.
